Needed to Register:

• Birth Certificate

• Social Security Card

• Up-to-date Immunization Record (or opt-out waivers)

• Proof of Residency in Arkansas (Driver's License, Bill, Lease Agreement)

All enrollment documents can be submitted by email. Send these documents to amorgan@icafortsmith.org along with your student's name in the subject line. You will be contacted once those forms are received.
